These Terms and Conditions ("Terms") govern your use of the Beamsville Smiles Family Dentistry website located at beamsvillesmiles.ca and the dental services provided at our practice. By accessing our website or booking an appointment, you agree to be bound by these Terms. If you do not agree, please do not use our website or services.
Beamsville Smiles Family Dentistry is a licensed dental practice operating at 4413 Ontario St Suite 203, Beamsville, Ontario, L3J 0A4. All dental services are provided by registered dental professionals governed by the Royal College of Dental Surgeons of Ontario (RCDSO) and applicable provincial legislation.
Appointments may be booked by phone, email, or via the contact form on our website. Booking a request online does not confirm an appointment, a member of our team will contact you to confirm availability and finalize your booking.
We reserve the right to refuse or reschedule appointments at our discretion, including in cases where clinical needs require a different level of care than we are equipped to provide.
We ask that you provide a minimum of 24 hours' notice if you need to cancel or reschedule your appointment. This allows us to offer that time to another patient who may be waiting for care.
Repeated late cancellations or no-shows may result in a cancellation fee or restriction on future booking. We understand that emergencies happen, please contact us as soon as possible if you are unable to attend.
Dental fees are based on the current Ontario Dental Association Suggested Fee Guide. A full breakdown of expected costs will be provided prior to any treatment.
Payment is due at the time services are rendered unless a prior arrangement has been made. We accept cash, major credit and debit cards, and direct insurance billing where applicable.
We are happy to direct-bill many dental insurance providers on your behalf as a courtesy. It is your responsibility to understand the limits and terms of your coverage. Any portion of fees not covered by your insurance plan remains your responsibility and is due at the time of service.
We cannot guarantee coverage or specific reimbursement amounts, as these are determined entirely by your insurance provider.
